About the Role:
Central London Community Healthcare NHS Trust (CLCH) seeks a dedicated Bank Ward Specialist Doctor to manage the medical care of patients in a rehabilitation ward in Hertfordshire. This role involves ongoing patient assessment, proactive medical intervention, and facilitating appropriate discharge. The doctor will be responsible for ensuring the optimal medical care of patients in their rehabilitation journey.
Key Responsibilities:
- Manage medical care of patients referred for rehabilitation.
- Manage medical deterioration and provide appropriate treatment options.
- Prevent unnecessary hospital readmissions through proactive daily patient reviews.
- Facilitate patient discharge to optimal locations, including home.
- Support patients, families, and carers.
- Clerk new patients and complete necessary assessments.
- Conduct daily reviews and ward sweeps.
- Review unwell patients escalated by nursing/therapy staff.
- Advise nursing/therapy staff on medical management.
Skills and Expertise:
- Specialist Doctor qualification (e.g. MRCP).
- Proven experience in managing patient care in a fast-paced environment.
- Excellent communication and interpersonal skills.
- Strong knowledge of medical protocols and procedures.
- Ability to work effectively within a multi-disciplinary team.
- Computer literacy (SystmOne or equivalent).
