About the Role:
We are seeking a highly experienced Senior Nurse to join our Rapid Access Chest Pain Clinic in London. The role involves supporting and enhancing the existing nurse-led clinic to ensure timely patient care and improved access for patients with chest pain, meeting Trust targets.
Key Responsibilities:
- Standardize and deliver high-quality patient care across sites.
- Expertly assess patients with chest pain, applying advanced nursing skills to identify and manage potential coronary artery disease.
- Operate within protocols, order investigations, interpret results, recommend appropriate treatments and present findings to medical staff.
- Utilize care pathways and risk stratification for early revascularization referral.
- Maintain clinical audit, adhering to National Service Framework and NICE guidelines.
- Contribute to the cardiology team and service development across the Trust.
- Educate patients on managing their symptoms and lifestyle modifications.
- Support and contribute to nurse-led Rapid Access Chest Pain clinic service.
- Assess patient cardiac care needs, utilizing advanced nursing skills in assessment and history taking.
- Assess CVD risk using NICE guidelines, empowering patients for improved health and wellbeing.
- Develop management plans for patients within the clinic’s remit, emphasizing health education and prevention.
- Assess and authorise patient admission or discharge.
- Provide oral and written information to patients and relatives about Coronary artery disease.
- Deliver health promotion regarding Coronary artery disease and its risk factors for primary and secondary prevention.
- Maintain accurate and compliant documentation.
- Act as a gateway for GP referrals, ensuring appropriate referrals to the RACPC.
- Inform GPs of clinic outcome and requested investigations.
- Recognize limitations and consult with other team members as needed.
- Improve service quality in the Rapid Access Chest Pain clinic.
- Act as an independent nurse prescriber adhering to NMC and Trust policy.
- Contribute to staff development within cardiology through teaching.
- Share knowledge with healthcare professionals in primary and secondary care through teaching and presentations.
- Develop and maintain clinical skills by attending relevant study days/conferences.
- Participate in staff and student orientation.
- Act in accordance with the NMC Code of Conduct.
- Exercise sensitivity and diplomacy in interactions with other healthcare professionals.
- Engage in personal clinical supervision and annual appraisal with line manager.
- Participate in clinical audit data collection, outcome measurement, and service development.
- Present audit results to the Trust and cardiac directorate.
- Apply research findings and other information to facilitate best practice.
- Actively participate in the Controls Assurance/Clinical Governance Group.
- Ensure clear, appropriate, and inclusive communication across all stakeholders.
Skills and Expertise:
- Significant experience as a Clinical Nurse Specialist
- Advanced cardiac assessment skills
- Strong knowledge of Coronary Artery Disease management
- Proven experience in using integrated care pathways
- Excellent communication and interpersonal skills
- Proficient in maintaining clinical records
- Ability to work autonomously and as part of a team
- Knowledge of NMC Code of Conduct
- Independent Nurse Prescribing ability
- Experience in clinical supervision and appraisal
- Experience with audit and research methods
