About the Role:
Deliver hospital discharge services for homeless patients at Gloucester and Cheltenham hospitals. Represent Elim’s values and contribute to the business plan, ensuring all services are delivered in line with the Psychologically Informed Framework and CARES values.
Key Responsibilities:
- Work onsite in Gloucester and Cheltenham hospital settings.
- Lead multi-agency collaboration with Local Authority and external agencies to reduce homelessness and improve patient outcomes.
- Monitor and review quality of support using outcome tools and systems, ensuring a psychologically informed approach.
- Manage budgets and service contracts, contributing to the annual budget setting process.
- Effectively utilize IT for service delivery, reporting, and outcome enhancement.
- Collect and report data monthly and quarterly to South Gloucestershire Council.
- Communicate and consult transparently with residents, staff, and stakeholders.
- Achieve individual and team KPIs, and contribute to departmental KPIs.
- Model and embed continuous improvement within services.
- Embrace change and contribute to service development.
- Complete mandatory and required training.
- Participate in appraisal and performance management.
- Work across services as needed to ensure support service delivery.
Skills and Expertise:
- Experience working with homeless patients.
- Knowledge of multi-agency working.
- Strong communication and interpersonal skills.
- Proficiency in relevant IT systems.
