About the Role:
This Practice Nurse role in South Yorkshire focuses on delivering high-quality general practice nursing care, including preventative health and evidence-based practice for patients with long-term conditions. The nurse will work autonomously, managing patient care and collaborating with the primary care multi-disciplinary team. This role offers significant opportunities for leadership within the practice.
Key Responsibilities:
- Assess, plan, develop, implement, and evaluate treatment programs for improved health and well-being.
- Assess, plan, implement, and evaluate individual treatment plans for patients with long-term conditions.
- Proactively identify, diagnose, and manage treatment plans for patients at risk of developing long-term conditions (e.g., COPD, Diabetes).
- Work with other healthcare professionals to diagnose, monitor, manage, and treat long-term conditions.
- Review medication for therapeutic effectiveness and patient needs, adhering to evidence-based practice.
- Support patient adherence to prescribed treatment.
- Provide information and advice on medication regimens, side effects, and interactions.
- Support patients in adopting health promotion strategies for healthy living.
- Support and manage health needs of women for family planning and cervical cytology.
- Implement and participate in vaccination and immunization programs for adults and children.
- Advise, support, and administer vaccinations for patients travelling abroad.
- Promote evidenced-based care for patients with aural conditions.
- Meet the needs of patients presenting for opportunistic wound care.
- Demonstrate sensitive communication styles for patient understanding and consent.
- Maintain effective communication with patients and carers, recognising alternative communication needs.
- Participate in practice team meetings, delivering the nursing agenda.
- Work within own competence and professional code of conduct, regulated by the NMC.
- Produce accurate, complete, and contemporaneous patient records.
- Prioritise and manage workload to maintain quality care.
- Deliver care according to NSF, NICE guidelines, and evidence-based practice.
- Assess care delivery effectiveness through self and peer review, benchmarking, and evaluation.
- Participate in quality governance systems and processes.
- Evaluate patient response to healthcare and care effectiveness.
- Participate in shared learning across the practice and wider organization.
- Participate in performance monitoring reviews, providing feedback.
- Understand and apply legal policies regarding vulnerable children and adults.
- Work within policies regarding family violence, vulnerable adults, substance misuse, and addictive behaviour.
- Interpret national strategies and policies into local implementation strategies.
- Take responsibility for own development, learning, and performance (including clinical supervision).
- Support the development of others to maximize staff potential.
- Promote the workplace as a learning environment.
- Participate in the planning and implementation of changes within care areas.
- Participate in the development of local guidelines, protocols, and standards.

- Ensure safe storage, rotation, and disposal of vaccines and drugs.
- Ensure monitoring, stock control, and documentation of controlled drug usage.
- Support members of the nursing team with mandatory and statutory training.
- Apply infection control measures according to local and national guidelines.
- Apply policies reducing environmental health risks and increasing access to healthcare.
- Utilize technology for planning, implementation, monitoring, and communication.
- Review and process data using accurate read codes for monitoring and audit.
- Manage information searches for patient information.
- Collate, analyze, and present clinical data to enhance care.
- Disseminate learning and information to other team members.
- Assess own learning needs and undertake learning as appropriate.
- Provide educational support to patients, carers, families, and colleagues.
- Identify patterns of discrimination and promote equality and diversity.
- Enable others to promote equality and diversity.

- Accept patients’ rights to choose their care providers and refuse care.
- Assist patients from marginalized groups to access quality care.
Skills and Expertise:
- Registered Nurse with NMC registration.
- Excellent communication and interpersonal skills.
- Strong clinical judgment and decision-making skills.
- Proficiency in patient assessment and care planning.
- Knowledge of relevant policies, procedures, and guidelines.
- Demonstrated experience with long-term conditions management.
- Experience in vaccination and immunisation.
- Strong understanding of safeguarding.
- Computer literacy and experience in electronic health records.
- Ability to work independently and collaboratively.
