About the Role:
This role requires an Advanced Nurse Practitioner to provide specialist palliative care in Cheshire East. The role will focus on inpatient, outpatient, and community care, ensuring holistic patient assessments and supporting patient and family needs.
Key Responsibilities:
- Deliver clinical reviews, admissions, and support safe discharges.
- Provide specialist palliative care input to outpatient/daycare services and community visits.
- Undertake holistic assessments using examination and diagnostic skills, including prescribing where appropriate.
- Support families and carers, liaise with healthcare professionals, and mutually agree upon care plans.
- Manage a caseload of patients with varying needs.
- Provide specialist advice and support to patients and carers, both face-to-face and via telephone.
- Coordinate care for palliative patients across East Cheshire in conjunction with specialist palliative care colleagues.
- Act as a role model and resource for other staff, and mentor clinical team members where appropriate.
- Maintain professional standards, uphold the hospice’s reputation, and update professional development.
- Support ongoing hospice development and quality assurance via audits, policies, procedures, and service reviews.
- Participate in research projects as required.
- Lead education development, planning, and delivery to peers and colleagues.
- Perform other duties commensurate with the post and grade.
Skills and Expertise:
- Advanced Nurse Practitioner qualification in Palliative Care.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Experience working with patients in various care settings.
- Knowledge of palliative care principles and practices.
- Prescribing experience (where applicable).
- Proficient in record-keeping and documentation.
- Ability to work independently and as part of a multidisciplinary team.
