About the Role:

This Senior Registered Nurse will be a proactive member of the Phyllis Tuckwell Hospice at Home team, providing holistic care to patients, families, and carers in their homes or care facilities. The role involves assessment, care planning, and symptom management for patients with palliative care needs, supporting them to live as fully as possible.

Key Responsibilities:

  • Assess, plan, coordinate, implement, and evaluate palliative care interventions.
  • Respond proactively to referrals and patient needs.
  • Ensure care is centred around the person, family, and carer.
  • Coordinate an effective nursing service, maintaining high standards of care.
  • Conduct face-to-face and telephone assessments to identify patient needs and arrange interventions.
  • Support palliative care colleagues and provide education.
  • Liaise with other healthcare professionals for continuity of care.
  • Manage patient workload, ensuring safety and clinical governance.
  • Provide optimal symptom management, psychological, and emotional support.
  • Assess changing patient needs and liaise with specialists.
  • Take on responsibilities for specific clinical areas, improving knowledge and practice.
  • Assess and support colleagues clinical competencies.
  • Attend and contribute to clinical multidisciplinary meetings.
  • Contribute to team cohesion and make autonomous decisions.
  • Adhere to NMC Code of Practice and Phyllis Tuckwell policies.
  • Represent Phyllis Tuckwell and maintain/enhance its reputation.
  • Maintain accurate patient records.
  • Develop expertise and act as a resource for the team.
  • Participate in inter-hospice meetings and maintain professional links with other agencies.
  • Identify and mitigate risks, reporting accidents or incidents promptly.
  • Adhere to medicine management policies.
  • Provide clinical support to relevant teams and external colleagues.
  • Maintain familiarity with relevant guidelines and regulations.
  • Ensure a safe working environment.
  • Report feedback, concerns, and complaints.
  • Adhere to Phyllis Tuckwells lone working policy.
  • Conduct 1:1 meetings and support new staff.
  • Provide clinical and personal support to team members.
  • Support a team approach through proactivity and positivity.
  • Maintain and develop knowledge/skills through CPD and appraisals.
  • Remain up to date on palliative care research and developments.
  • Participate in research and audit within Phyllis Tuckwell.
  • Ensure audits are completed, results communicated, and actions implemented.
  • Adhere to Phyllis Tuckwells Safeguarding Policy.
  • Inform appropriate PT professionals of safeguarding concerns.

Skills and Expertise:

  • Registered Nurse with extensive experience.
