About the Role:

This role involves delivering specialist palliative care to patients in Cheshire East, including inpatient, outpatient, and community settings. A key responsibility includes supporting safe discharges and holistic assessments, ensuring patient and family needs are met.

Key Responsibilities:

  • Work as part of a medical team on the inpatient unit, performing clinical reviews and supporting safe discharges.
  • Provide specialist palliative care input to outpatient/daycare services and undertake community visits.
  • Undertake holistic assessments of patients using examination and diagnostic skills, including prescribing where appropriate.
  • Support families and carers, liaise with healthcare professionals, and mutually agree upon care plans.
  • Manage a caseload of patients with varying needs, providing specialist advice and support both in person and via telephone.
  • Coordinate care for palliative patients across East Cheshire.
  • Act as a role model and resource for other staff, supporting team members and mentoring clinical colleagues where appropriate.
  • Maintain professional development, knowledge, skills, and competencies, ensuring adherence to professional standards.
  • Support hospice development and quality assurance measures via audits, policies, procedures, and service reviews.
  • Participate in research projects as required.
  • Develop, plan and deliver education to peers and colleagues.
  • Undertake other duties commensurate with the post and grade.
  • Comply with control of infection prevention and management protocols.

Skills and Expertise:

  • Band 7 Advanced Nurse Practitioner qualification.
  • Extensive experience in palliative care.
  • Excellent communication and interpersonal skills.
  • Strong diagnostic skills and ability to prescribe.
  • Experience in managing patient caseloads.
  • Proven ability to work collaboratively within a multidisciplinary team.
  • Excellent communication skills with patients, families, and healthcare professionals.
  • Knowledge of hospice policies and procedures.
  • Commitment to professional development.
  • Strong organizational and time management skills.
