Key Responsibilities:

  • Deliver advanced clinical practice within the defined scope of practice, taking accountability for decisions and actions.
  • Undertake comprehensive multi-needs assessments using advanced clinical reasoning to develop patient-centered treatment plans.
  • Act as a source of expert opinion for other healthcare professionals.
  • Promote active patient and carer participation in care.
  • Communicate complex clinical information sensitively and effectively, adapting to cultural and socioeconomic factors.
  • Demonstrate advanced sensory and physical skills in examinations and assessments, utilizing expertise in clinical procedures and treatments.
  • Use clinical reasoning to make evidence-informed judgments and diagnoses.
  • Initiate, evaluate, and modify interventions, including prescribing medicines and advanced treatments.
  • Manage risk in complex situations, supporting teams in doing the same.
  • Collaborate with multi-agency and multi-professional teams to maintain and evaluate service links.
  • Act as a clinical role model, advocating for responsive care.
  • Order investigations, provide individual and team-based treatment and care, and refer patients appropriately.
  • Admit or discharge patients, and refer to other healthcare providers within the scope of practice.
  • Investigate complaints relating to patient care, develop action plans, and propose changes to practice.
  • Apply national guidelines and legislation to local service provision.
  • Support people in decision-making, care planning, and positive change using a person-centered approach.
  • Contribute to clinical governance and quality assurance programmes.
  • Develop guidelines, protocols, and innovative solutions, collaborating with colleagues.
  • Lead and manage staff, providing guidance, supervision, and performance management.
  • Participate in the recruitment, selection, and retention of staff, working with clinical leads and HR.
  • Inform the service of changes in clinical practice to support service delivery, business planning, and strategic reviews.
  • Provide clinical consultancy to enhance clinical reasoning and decision-making.
  • Promote best practice sharing and adoption.
  • Develop and maintain effective relationships within the trust and wider health economy.
  • Evaluate own practice and participate in multi-professional evaluations.
