About the Role:

Oxleas NHS Foundation Trust seeks a dynamic Advanced Clinical Practitioner to join the Bexley Rapid Response/Urgent Community Response Team (UCR). This role offers clinical leadership and line management within a multi-professional team, focusing on reducing unnecessary hospital admissions and supporting patient needs in the community. The role will be key to shaping the future of UCR services in Bexley.

Key Responsibilities:

  • Provide advanced clinical assessment and care to patients experiencing crisis ill health.
  • Lead clinical decision-making and contribute to service improvement strategies.
  • Manage and support junior colleagues within the multi-professional team.
  • Utilize non-medical prescribing qualifications to medically support complex patients in the community.
  • Implement evidence-based clinical changes and lead training/education for the team.
  • Identify and address service gaps to improve patient outcomes.
  • Ensure a positive team culture through effective leadership.

Skills and Expertise:

  • Extensive clinical experience in the management of patients.
  • Advanced clinical assessment skills.
  • Experience leading community teams.
  • Strong leadership, communication, and interpersonal skills.
  • Non-medical prescribing qualification (desirable).
  • Demonstrated critical thinking and problem-solving skills.
  • Knowledge of national and local healthcare strategies.
