About the Role:

This Advanced Clinical Practitioner (ACP) role focuses on providing highly specialized advice and support to patients, GPs, and healthcare teams in the Surgical Receiving Unit. Key responsibilities include managing complex information, facilitating informed decision-making, and coordinating patient care across various departments. The role requires strong communication skills and experience with patients and families, as well as significant knowledge of clinical procedures.

Key Responsibilities:

  • Communicate complex information regarding urological conditions, risks, and benefits to patients and families.
  • Provide highly specialist advice to GPs and primary health care teams.
  • Support patients, carers, and family members in accessing and understanding information.
  • Conduct family meetings to facilitate communication and care planning.
  • Act as a clinical leader, managing workload and improving services.
  • Participate in networks and professional development opportunities.
  • Assess and manage risk, challenging others about risk factors.
  • Participate in recruitment and selection processes.
  • Support patients and families through emotional challenges.
  • Coordinate care transitions between different care centres.
  • Implement systems to demonstrate the impact of advanced nursing.
  • Provide support to other nurses and professional groups on therapeutic interventions.

  • Support other Specialist nurses in their prescribing confidence.
