About the Role:
As a Care Coordinator for Shared Lives Oldham, you’ll play a pivotal role in supporting adults with varying needs and their carers. You will work closely with a dedicated team and commissioners to ensure appropriate placements within the Shared Lives Oldham scheme.
Key Responsibilities:
- Support Shared Lives Carers through contact and visits, addressing any issues arising.
- Conduct thorough assessments of potential Shared Lives Carers, producing reports for the Approval Panel.
- Support Shared Lives Carers to meet service requirements, legislation (Health & Social Care Act, Care Act, CQC guidelines) and personal care provision.
- Participate in training events, carer meetings, and social activities, fostering engagement and consultation.
- Collaborate with care management, individuals, and carers/families to develop comprehensive care plans and provide support.
Skills and Expertise:
- Experience working with adults with diverse needs.
- Understanding of care legislation (Health & Social Care Act 2008, Care Act 2014, CQC guidelines).
- Excellent communication and interpersonal skills.
- Strong problem-solving and analytical abilities.
- Ability to build rapport with service users and carers.
- Proficiency in report writing.
Job Facilities/Benefits:
- Salary from 30,060 to 33,366 per year.
- Full-time employment (36.66 hours per week).
- Based at Ena Hughes Resource Centre, Failsworth.
- Borough-wide travel may be required for caseload.
