Key Responsibilities:
- Agree a definitive list of minor illness conditions with Lead Clinicians.
- Communicate prescribing messages to practice staff.
- Review medication use associated with unplanned hospital admissions and readmissions.
- Monitor practice and make recommendations based on audits.
- Audit compliance against NICE and JRCALC guidance.
- Provide education and training to the healthcare team.
Skills and Expertise:
- Pharmacist qualifications.
- Proven experience in clinical pharmacy.
- Strong communication skills.
- Knowledge of NICE and JRCALC guidelines.
- Ability to conduct audits and reviews.
- Experience working with healthcare teams.
