About the Role:

Support general practice and Primary Care Network (PCN) staff with medication queries, medicines reconciliation, and safer prescribing. Provide clinical leadership in medicines optimisation and quality improvement, impacting patient care within the PCN.

Key Responsibilities:

  • Provide primary support to general practice staff on prescription and medication queries.
  • Perform medicines reconciliation on transfer of care.
  • Implement systems for safer prescribing.
  • Provide clinical medicines advice addressing patient needs.
  • Work as part of a multi-disciplinary team.
  • Take responsibility for medicines optimisation within the PCN.
  • Undertake structured clinical medication reviews.
  • Provide clinical leadership on medicines optimisation and quality improvement (QI).
  • Manage aspects of the quality and outcomes framework and enhanced services.
  • Develop and quality-assure repeat prescribing and medication review processes.
  • Conduct focused reviews of repeat prescriptions.
  • Offer medication review services via clinics, domiciliary visits, and residential/nursing homes.
  • Undertake disease-focused medication reviews, particularly for high-risk groups.
  • Ensure appropriate compliance with national/local guidelines for high-risk medications.
  • Provide telephone advice to patients with long-term conditions.
  • Be familiar with and contribute to practice ICB long-term conditions pathways.
  • Provide health promotion, including flu vaccinations.
  • Conduct initial assessments of common general practice presentations.
  • Independently diagnose and manage minor illnesses.
  • Review and reconcile medicines changes from secondary care.
  • Reconcile patient records with discharge medication.
  • Ensure suggested actions from hospital discharge letters are followed through.
  • Manage practice formularies and IT systems for medication selection.
  • Facilitate the development and usage of electronic prescribing.
  • Update and maintain accurate patient medication records.
  • Ensure appropriate medicines governance.
  • Lead the response to drug safety alerts and clinical guidance changes.
  • Assist with medicines management guidance and policy development.
  • Advise primary healthcare team on controlled drug handling.
  • Identify and flag medicines management risks (clinical and financial).
  • Offer advice on adverse events involving medication.
  • Offer advice on patient complaints related to medication/prescribing.
  • Plan and undertake clinical/prescribing audits.
  • Provide regular support and feedback on prescribing action plans.
  • Report on practice prescribing figures with benchmarking.
  • Analyse practice complex prescribing data.
  • Lead on medicines management targets and initiatives (QOF, QIPP, CCG).
  • Support the implementation of audit findings.
  • Provide training in medicines issues to other practice members.
  • Develop plans for training the reception team.
  • Analyse and appraise complex information for clinicians.
  • Prepare evidence-based resources and information.
  • Present at patient group meetings or appropriate events.
  • Produce patient information leaflets and posters.
  • Raise awareness of out-of-practice schemes.
