About the Role:
This role involves delivering evidence-based nursing care to children and young people in Bolton. The successful candidate will be responsible for clinical assessment, planning, and evaluation of care, collaborating with multidisciplinary professionals. The role focuses on preventing illness and promoting health outcomes for children and young people within the community.
Key Responsibilities:
- Deliver evidence-based nursing care to children and young people (CYP) in community settings.
- Conduct clinical assessments, plan, deliver, and evaluate care.
- Monitor acute exacerbations of illness and initiate appropriate nursing support/urgent medical interventions.
- Collaborate with hospital inpatient areas, A&E, and primary care to facilitate timely discharge.
- Collaborate with multidisciplinary teams to ensure patient-centered, efficient, and effective care.
- Develop guidelines and policies for children with acute and continuing health needs.
- Utilize health promotion skills and implement self-management initiatives.
- Act as patient advocate, promoting highest standards of care.
- Support named nurses with designated caseloads for CYP and families.
- Collaborate with other professionals and agencies in the assessment, implementation, and evaluation of specialist care.
Skills and Expertise:
- Registered Nurse with experience working with children and young people.
- Strong assessment, planning, and evaluation skills.
- Knowledge of relevant legislation and policies.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in multidisciplinary teams.
- Experience in providing preventative care and promoting health.
- Proactive approach to resolving issues and problem-solving skills.
- Self-motivated and highly organized.
