About the Role:

This role within a 24/7 Community Nursing Team involves assessing, planning, implementing, and evaluating patient care. The successful candidate will support the strategic direction of service delivery across PCCS.

Key Responsibilities:

  • Assessing, planning, implementing, and evaluating patient care under the direction of the Team Leader.
  • Making judgments on patient conditions and developing care programs.
  • Demonstrating competence in clinical skills (IV additives, suprapubic catheterisation, cannulation, venepuncture, etc.).
  • Accepting professional responsibility for first visit and delegated care.
  • Ensuring the quality of care delivered to patient caseloads and participating in formal standard monitoring.
  • Maintaining accurate records/standards for documentation, including IT.
  • Participating in clinical audit and data collection.
  • Contributing to the development and delivery of the corporate nursing agenda.
  • Undertaking any other duties consistent with the overall grade/responsibilities of the post.
  • Acting as a mentor/preceptor to students and junior staff.
  • Contributing to meeting the learning needs of team members.
  • Sharing expertise and evaluating training undertaken.
  • Improving current practices by promoting research-based practice.
  • Planning the provision of care for patients.
  • Ensuring services are provided safely and sustainably.
  • Adhering to Health Board procedures: Health and Safety at Work Act, Data Protection Act 1998, and other relevant legislation.
  • Providing leadership and supervision of junior staff, support workers, and students.
  • Deputizing in the absence of the Team Leader/Deputy.
  • Being aware of budgetary implications when prescribing care.
  • Contributing to the annual Personal Reviews of junior colleagues.
  • Identifying training needs and developing Personal Development Plans.
  • Being familiar and complying with Health Boards Policies and Procedures.
  • Successfully managing the introduction of changes in practice within the team.
  • Attending and participating in team meetings.
  • Being aware of and participating in the delivery of the Divisional Clinical Governance agenda.
  • Contributing to the recruitment and selection process.

Skills and Expertise:

  • Strong clinical skills and experience.
  • Excellent communication skills, especially with patients with special needs.
  • Experience in assessing, planning, and evaluating patient care.
  • NMC Professional Code of Conduct adherence.
  • Proven ability to liaise with multidisciplinary teams.
  • Knowledge of relevant health legislation and policies.
  • IT proficiency for accurate record-keeping.
  • Mentoring and leadership skills.
  • Ability to work effectively within a team environment.
