About the Role:

As a Band 5 Community Nurse at Melksham Community Hospital in Wiltshire, you’ll be a valued member of our team, supporting patients within the community. This role focuses on delivering high-quality care and contributing to the overall well-being of our patients.

Key Responsibilities:

  • Provide direct patient care, including assessments, treatment plans, and monitoring.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Document patient progress and communicate effectively with colleagues.
  • Adhere to relevant NHS policies and procedures.
  • Contribute to the overall aims and objectives of the community nursing team.

Skills and Expertise:

  • Registered Nurse with current NMC registration.
  • Experience working with patients in the community setting.
  • Excellent communication and interpersonal skills.
  • Strong clinical judgment and problem-solving abilities.
  • Knowledge of relevant NHS policies and procedures.
