About the Role:

Provide vital community healthcare services as a flexible Community Psychiatric Nurse, impacting patients’ lives while balancing professional and personal commitments. This role allows for a unique blend of professional fulfilment and adaptable scheduling, offering a satisfying work-life balance.

Key Responsibilities:

  • Assess patients (telephone or face-to-face) presenting with mental health difficulties.
  • Identify appropriate care pathways and ensure least restrictive options.
  • Signpost to appropriate care providers (primary and tertiary).
  • Raise concerns to senior staff for support and management.
  • Complete documentation and care planning.
  • Present cases to Multi-Disciplinary Team for care pathway discussion.
  • Liaise with colleagues (nursing, occupational therapy, medics, psychology, and admin).

Skills and Expertise:

  • Minimum 6 months paid experience as a Community Psychiatric Nurse in a UK hospital (within the last 3 years).
  • Compliance with NMC Professional Registration.
  • Up-to-date knowledge of current nursing practices.
  • Full UK driving licence and own vehicle.
  • (Variable additional certifications/skills may be required, depending on the Trust)
