About the Role:
Provide skilled nursing care to adults and their families in the First Community (FC) setting, focusing on promoting health, wellbeing, and independence. You will work within a team and support the Senior Nurse, possibly overseeing care in their absence.
Key Responsibilities:
- Work within a rota to cover service needs, 52 weeks a year.
- Plan and monitor person-centred health care, demonstrating clinical decision-making in records.
- Participate in patient admission and discharge planning.
- Promote patient wellbeing, health promotion, and self-management.
- Adhere to FC and local policies, guidelines, procedures, and SOPs.
- Promote effective communication within FCs behaviour framework.
- Supervise and delegate care to junior staff, maintaining accountability.
- Escalate concerns to the senior nurse.
- Maintain records according to FC policy and NMC Guidelines, including EMIS.
- Work independently and collaboratively within the wider Health and Social Care Networks.
- Maintain professional and clinical integrity in line with NMC Code.
- Effectively engage with all team members involved in patient care.
- Support induction, supervision, and teaching of junior team members (including students and temporary staff).
- Act as an advocate and role model in the community setting.
- Understand budgetary constraints and manage resources effectively.
- Ensure adherence to local and national Health and Safety regulations.
- Report and escalate risks, following FCs incident reporting process.
- Protect vulnerable individuals, adhering to relevant procedures.
- Attend and engage in organisational meetings, providing feedback to the team.
- Participate in clinical supervision and reflective practice sessions.
- Assist in service audits to support ongoing review of effective practice.
- Escalate staffing issues impacting safe and effective care, in the absence of the senior nurse.
Skills and Expertise:
- Registered Nurse with NMC registration
- Proven experience in adult nursing
- Strong communication and interpersonal skills
- Excellent problem-solving and decision-making skills
- Experience supervising junior staff
- Proficiency in electronic patient records systems (EMIS)
