About the Role:
Our team is seeking a dedicated and experienced Community Staff Nurse to provide specialized wound care and lymphedema management. This role plays a critical role in the care of patients in their homes, focusing on wound healing and preventing complications.
Key Responsibilities:
- Assess and treat wounds and lymphedema, adhering to best practice guidelines.
- Develop and implement individualised care plans for patients.
- Monitor patient progress and adjust care strategies as needed.
- Collaborate with multidisciplinary teams including GPs, specialists and other healthcare professionals.
- Educate patients and their families on wound care and lymphedema management.
- Maintain accurate records and documentation of patient care.
Skills and Expertise:
- Registered Nurse with current UK Nursing Registration.
- Proven experience in wound care and/or lymphedema management.
- Excellent communication and interpersonal skills.
- Knowledge of relevant legislation and guidelines.
- Strong organizational and time management skills.
- Ability to work independently and as part of a team.
