About the Role:

Wrightington, Wigan and Leigh Teaching Hospitals NHS Foundation Trust seeks a passionate and experienced Consultant Practitioner to spearhead the development of our Frailty SDEC Unit and co-located Frailty Short Stay Unit. This key role focuses on providing advanced assessment, diagnosis, and management of frailty patients, collaborating with a dedicated multidisciplinary team.

Key Responsibilities:

  • Deliver expert clinical assessment, examination, and diagnosis for frailty patients across the community and hospital.
  • Manage and implement treatment plans, including initiating investigations and interpreting results.
  • Develop and lead advanced clinical care practice standards, supervision, and governance for a future team of ACPs within frailty services.
  • Collaborate with ED and Community Response Team (CRT) for timely patient transfers to the Frailty SDEC.
  • Develop trust-wide frailty policies, guidelines, and pathways.
  • Contribute to the development and implementation of a proactive care system focused on admission avoidance.
  • Collaborate with local partners including Primary Care, Social Services, Mental Health, and the Voluntary sectors.
  • Lead and deliver educational programs on frailty and older adults care.
  • Champion the delivery of safe, high-quality care for frail patients.
  • Develop and implement evidence-based protocols, guidelines, and SOPs for improved care.
  • Strategically promote frailty within the trust and across community partners.
  • Provide expert clinical supervision to support staff development.

Skills and Expertise:

  • Advanced practitioner experience with a non-medical prescribing qualification (NMP).
  • Expert skills in assessment, examination, and providing diagnosis/management for patients across the frailty pathway.
  • Proven ability to make independent clinical decisions.
  • Knowledge of specialist investigations (e.g., blood tests, scans).
  • Excellent communication, interpersonal, and teamwork skills.
  • Experience in developing and implementing clinical pathways.
  • Experience leading and mentoring others.
  • Demonstrated experience in critical thinking, research-based practice and applying evidence to clinical practice.
