About the Role:
Promote the health and wellbeing of the North Yorkshire population. Deliver a blended service approach to families, with both face-to-face and virtual contacts. Advocate for clients and communities to ensure physical and mental health needs are met.
Key Responsibilities:
- Prevent ill health and promote positive holistic healthcare.
- Plan, organise, and deliver care programmes in conjunction with families and multi-disciplinary teams.
- Conduct continuous assessment of children, young people, and families.
- Provide evidenced-based service to children, families, and communities in line with national and local policies.
Skills and Expertise:
- Excellent communication skills
- Passion for public health
- Good organisational skills
- Flexibility to meet service needs
- Ability to travel independently for service delivery
