About the Role:

Key Responsibilities:
- Provide individualised care to patients with long-term conditions.
- Complete nursing assessments and follow treatment regimes.
- Work collaboratively in a multi-professional team.
- Think proactively and be empathetic.
- Communicate effectively with staff and patients.
- Ensure behaviours and values align with CLCH.
- Contribute to weekend rota (typically 3 weekend days per 4-week cycle).
Skills and Expertise:
- Qualified Nursing Associate (NA) or equivalent.
- Previous community nursing experience (preferred).
- Strong clinical skills and experience.
- Excellent teamwork and communication skills.
- Willingness to learn and develop.
- Driving license and access to a vehicle.
