About the Role:
This role combines the expertise of an Occupational Therapist with the supportive role of a Keyworker. The key purpose of the position is to provide comprehensive care and support to individuals, ensuring their well-being and engagement in their community.
Key Responsibilities:
- Assess and develop individualised care plans for clients.
- Implement interventions tailored to meet specific needs.
- Collaborate with multidisciplinary teams to provide holistic support.
- Monitor progress and adjust interventions as needed.
- Promote independence and participation in daily activities.
- Support clients in accessing community resources and services.
- Document and report on client progress.
Skills and Expertise:
- Registered Occupational Therapist with relevant UK experience.
- Proven ability to build strong rapport with clients and their families.
- Excellent communication, interpersonal, and problem-solving skills.
- Strong understanding of mental health principles and practices.
- Knowledge of relevant legislation and guidelines.
- Computer literacy and proficiency in relevant software.
