About the Role:
We are seeking an Outreach Worker to join our Hillingdon Outreach Team, supporting rough sleepers in the Heathrow area. This role involves assessing needs, advising on options, and linking them with appropriate external services. It is a challenging yet rewarding position requiring flexibility and a commitment to empowering individuals.
Key Responsibilities:
- Work in pairs and independently to support rough sleepers with their needs.
- Assist rough sleepers in accessing social care, accommodation, and other support services.
- Conduct needs and risk assessments for new rough sleepers.
- Update assessments and the CHAIN database for existing rough sleepers.
- Identify and implement the best course of action for each client’s situation.
- Collaborate with various agencies, including local authorities, health services, housing providers, police, and drug/alcohol services.
Skills and Expertise:
- Excellent interpersonal and communication skills.
- Ability to build strong working relationships with rough sleepers.
- Experience in dealing with vulnerable individuals (homeless experience preferred).
- Flexibility to work varied shifts, including evenings and overnight.
- Strong IT skills for data collection, collation, and analysis.
- Confidence in representing the organization at external meetings.
- Enhanced DBS check.
