About the Role:
This Paramedic Practitioner role in West Sussex provides first-contact clinical care to patients in the practice or their homes. The role involves a variety of consultations, including in-person, remote, telephone, and video. Working within a Primary Care Network, you will play a critical role in delivering timely and effective patient care.
Key Responsibilities:
- Provide assessment, diagnosis, and treatment of patients at first point of contact.
- Work collaboratively with practice teams to ensure smooth patient referrals, triaging, and prioritizing patients.
- Accurately triage and prioritize patients, including in emergency situations.
- Evaluate clinical information, initiate appropriate treatment and/or referrals.
- Interpret diagnostic results and take action to manage patient care.
- Apply clinical guidelines (e.g., NICE) to individual patient cases.
- Provide assistance and feedback to the organization on clinical guidelines interpretation.
- Signpost patients to other healthcare services within the healthcare pathway.
- Advise and support patients on health promotion and disease prevention.
- Maintain accurate documentation of patient interactions and care.
- Support the delivery of anticipatory care plans and lead certain services (e.g., monitoring blood pressure and diabetes risk).
- Recognize and address inequalities and unwarranted variation in healthcare.
- Actively participate in practice and clinical meetings.
- Ensure high-quality communication with patients and carers, including in challenging situations.
- Proactively support the audit of caseloads to maintain and improve service quality.
- Seek feedback from other members of the MDT and patients.
- Maintain compliance with all policies, procedures, and training requirements.
- Ensure practice is within good clinical governance and based on sound clinical evidence.
- Identify and assess risks associated with patient care and communicate them to the appropriate clinical manager.
- Participate in staff and student education and training.
- Maintain continuous professional development through courses and study days.
- Complete all statutory and mandatory training.
Skills and Expertise:
- Registered Paramedic Practitioner with current UK registration.
- Strong clinical assessment and diagnostic skills.
- Proven experience in patient care and communication.
- Excellent knowledge of clinical guidelines and procedures.
- Experience with various clinical systems and technologies.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proven commitment to patient care and continuous professional development.
