About the Role:
The Harness South PCN Pharmacy Technician is a pivotal role within a dynamic Clinical Pharmacy team. The position offers an excellent opportunity for professional development and collaboration within a valued team supporting multiple GP practices.
Key Responsibilities:
- Provide professional support and clinical expertise to general practice teams and Clinical Pharmacists on medication queries, prescribing issues, and safe prescribing systems.
- Support Clinical Pharmacists with Clinical Medication Reviews and Structured Medication Reviews, including Care Home and Domiciliary Reviews.
- Support practice teams in achieving local medicines optimisation schemes and QOF targets.
- Participate in the delivery of long-term condition clinics and medicines reconciliation.
- Provide medicines information to practice staff and patients, and offer signposting for public and social care needs.
- Liaise with providers (community and hospital pharmacists) for effective medication management.
- Provide pharmaceutical support to practices, reviewing medication processes, applying best practice, and embedding evidence-based values.
- Ensure optimum benefit for prescribed medication through patient and carer support.
- Support Clinical Pharmacists in direct patient care for long-term conditions and other clinical areas.
- Support complex patient care in homes or residential accommodation.
- Utilize Pharmacy Technician skills for cost-effective NHS resource management and waste reduction.
- Provide professional leadership on medicines optimisation and quality improvement.
- Apply clinical expertise in supporting practice teams to deliver Clinical Level 3 Medication Reviews/ Structured Medication Reviews.
- Proactively reduce inappropriate polypharmacy and over-diagnosis through de-prescribing.
- Support practices with achieving QOF and local medicines optimisation targets.
- Promote public health, educate patients on healthy choices, and facilitate access to relevant services.
- Participate in MDT meetings for optimal patient care.
- Provide telephone/email advice and support to patients and carers.
- Reconcile patient medications during care transitions, liaising with community pharmacies.
- Maintain patient records using clinical templates and SNOMED coding.
- Review medication needs and provide recommendations to Clinical Pharmacists and GPs.
- Manage acute medication requests in line with local standards.
- Develop systems for safe and effective repeat prescribing and reducing wastage.
- Ensure continuous medication supply to high-risk patients.
- Implement changes to medicines based on MHRA alerts and local/national guidance.
- Support practices to implement relevant NICE guidelines.
- Assess and counsel patients on medication usage techniques.
- Assist with discharge communications.
- Ensure medication records are up-to-date with interventions.
- Take monitoring observations and record in patient notes.
- Engage in audit, research, and risk stratification activities, including high-risk patient identification.
- Provide feedback and implement recommendations from clinical audit activities.
- Engage with research activities at the Harness Education and Research Hub.
- Analyse, interpret, and present prescribing information for decision-making.
- Support the development of integrated care in Brent.
- Adhere to Harness policies and procedures, including information governance, equal opportunities, safeguarding, communications, lone worker, health & safety, and infection control.
- Support public health campaigns at relevant levels.
- Work with practices to ensure full compliance with Care Quality Commission standards.
- Participate as a member of the Harness multidisciplinary team.
- Participate in professional development activities.
Skills and Expertise:
- Experience in General Practice (desirable, not essential)
- Self-motivated and autonomous worker
- Excellent written and verbal communication skills
- Knowledge of clinical pharmacy principles and practices
- Proficiency in relevant software and systems
- Understanding of medicines optimisation techniques
- Experience in medication reviews (desirable)
- Knowledge of QOF targets and local medicine optimisation schemes
- Awareness of clinical guidelines and best practices
- Ability to work flexibly, both in-practice and remotely
