About the Role:

We are seeking a skilled and compassionate Practice Nurse to provide comprehensive care to patients in Gloucestershire. This role is focused on developing and implementing effective care management plans for patients, promoting health and wellbeing, and delivering high-quality clinical services.

Key Responsibilities:

  • Develop, implement, and embed care management programmes for frail and housebound patients.
  • Assess patient needs and ensure appropriate healthcare provision aligned with evidence-based practice.
  • Develop and implement health promotion and wellbeing programs.
  • Implement and evaluate individual treatment plans for chronic disease patients.
  • Identify, manage, and support patients at risk of long-term conditions.
  • Provide routine nursing care, including venepuncture, wound care, and ECGs.
  • Diagnose and manage acute and chronic conditions as required.
  • Prescribe medication according to PGDs and scope of practice.
  • Review patient medications to enhance compliance.
  • Provide guidance to patients on prescribed and over-the-counter medications.
  • Coordinate travel medicine services.
  • Request and process pathology services.
  • Participate in chronic disease clinics and patient care referral as necessary.
  • Maintain accurate clinical records.
  • Ensure effective use of read codes and chronic disease registers.
  • Develop, implement, and participate in well woman and well man clinics.
  • Adhere to the practice chaperone policy.
  • Assist GPs with minor surgery as required.
  • Prioritize health issues and intervene appropriately.
  • Support the team in clinical emergencies.
  • Recognise, assess, and refer patients with mental health needs.
  • Implement vaccination programs.
  • Support patients in medicine use (within scope of practice).
  • Liaise with external services for patient support (vulnerable patients).
  • Delegate clinical responsibilities, ensuring safe practice and scope of practice.
  • Support the clinical team with safeguarding matters.
  • Understand practice policies on substance abuse and addictive behaviour.
  • Deliver opportunistic health promotion.
  • Monitor fridge and stock temperatures and expiry dates.
  • Develop and review audit protocols and processes.
  • Develop, review, and implement nursing protocols.
  • Support, guide, and conduct appraisals/training for nursing team members.
  • Participate in local initiatives to improve service delivery.
  • Support and participate in shared learning for improved patient care.
  • Review clinical practices in response to national policies/initiatives.
  • Participate in significant/near-miss event reviews (root cause analysis).
  • Coordinate nursing team rotas.
  • Lead planning and implementation of changes within the nursing team.
  • Support practice team with policy review and implementation.

Skills and Expertise:

  • Registered Nurse (RN) qualification
  • Experience in managing patients with chronic conditions, especially diabetes.
  • Excellent communication and interpersonal skills.
  • Strong organizational and time management skills.
  • Proficient in using clinical systems and software.
  • Ability to work independently and collaboratively within a team.
  • Understanding of patient safeguarding policies.
