About the Role:
This exciting opportunity welcomes a highly motivated Primary Care Staff Nurse to join our expanding Health & Justice team at HMP High Down in London. The role focuses on providing responsive and high-quality primary care to patients within a secure environment, aiming to promote health and well-being.
Key Responsibilities:
- Provide individual patient assessment, treatment, and referral services.
- Monitor patient health status and address identified issues.
- Collaborate with patients to influence health outcomes based on lifestyle patterns.
- Develop effective discharge plans for transfer or release locations.
- Coordinate multi-disciplinary clinical practice to ensure integrated care.
- Collaborate with officers and governors to create coordinated treatment plans.
- Triage patient needs according to standard operating procedures and national policy.
- Assess, plan, and implement patient care within specific care pathways.
- Employ evidence-based practice to deliver effective and appropriate clinical care.
- Perform general nursing duties.
- Maintain accurate and contemporaneous records.
- Contribute to the development of integrated healthcare.
- Act as a preceptor/mentor as appropriate.
Skills and Expertise:
- Qualified Registered Nurse
- Experience in primary care settings (preferred)
- Excellent communication and interpersonal skills
- Ability to work collaboratively in multi-disciplinary teams
- Knowledge of relevant legislation and guidelines
- Strong problem-solving and decision-making skills
- Ability to work under pressure in a secure environment
