About the Role:

This exciting opportunity welcomes a highly motivated Primary Care Staff Nurse to join our expanding Health & Justice team at HMP High Down in London. The role focuses on providing responsive and high-quality primary care to patients within a secure environment, aiming to promote health and well-being.

Key Responsibilities:

  • Provide individual patient assessment, treatment, and referral services.
  • Monitor patient health status and address identified issues.
  • Collaborate with patients to influence health outcomes based on lifestyle patterns.
  • Develop effective discharge plans for transfer or release locations.
  • Coordinate multi-disciplinary clinical practice to ensure integrated care.
  • Collaborate with officers and governors to create coordinated treatment plans.
  • Triage patient needs according to standard operating procedures and national policy.
  • Assess, plan, and implement patient care within specific care pathways.
  • Employ evidence-based practice to deliver effective and appropriate clinical care.
  • Perform general nursing duties.
  • Maintain accurate and contemporaneous records.
  • Contribute to the development of integrated healthcare.
  • Act as a preceptor/mentor as appropriate.

Skills and Expertise:

  • Qualified Registered Nurse
  • Experience in primary care settings (preferred)
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively in multi-disciplinary teams
  • Knowledge of relevant legislation and guidelines
  • Strong problem-solving and decision-making skills
  • Ability to work under pressure in a secure environment
