About the Role:

An exciting opportunity has arisen for a Band 5 Registered Nurse to join a dedicated team on Ward C6, a 21-bed elderly medical ward. This role focuses on providing high-quality care for patients with acute and chronic medical conditions.

Key Responsibilities:

  • Manage care for a group of patients, ensuring effective workload and care priority management.
  • Provide high standards of care, adhering to quality and best practice guidelines.
  • Assess patient care needs, plan, deliver, and evaluate nursing care.
  • Ensure safe delivery of prescribed medications and monitor effectiveness.
  • Monitor patient clinical condition using tools and clinical judgment, escalating issues as required.
  • Implement appropriate harm reduction measures to maintain patient safety.
  • Work effectively within the multidisciplinary team (MDT), advocating for patients.
  • Ensure safe admission, transfer, discharge, and handover of patient care.
  • Communicate effectively with patients and their families, respecting their wishes.
  • Demonstrate knowledge of legal frameworks related to consent, capacity, safeguarding adults, and vulnerabilities.
  • Maintain clear and concise records.
  • Supervise qualified and unqualified staff, contributing positively to their professional development.
  • Undertake the role of shift leader once trained and competent.

Skills and Expertise:

  • Registered Nurse qualification (Band 5).
  • Experience in providing care for elderly patients.
  • Excellent communication and interpersonal skills.
  • Strong clinical judgment and problem-solving skills.
  • Proven ability to manage patient care effectively.
  • Knowledge of local policy and procedure.
  • Understanding of legal frameworks related to consent, capacity, safeguarding adults, and vulnerabilities.
  • Proficient in care documentation.
  • Experience in medication administration.
  • Knowledge of managing patients with multiple long-term health conditions and acute conditions.
