About the Role:
An exciting opportunity has arisen for a Band 5 Registered Nurse to join a dedicated team on Ward C6, a 21-bed elderly medical ward. This role focuses on providing high-quality care for patients with acute and chronic medical conditions.
Key Responsibilities:
- Manage care for a group of patients, ensuring effective workload and care priority management.
- Provide high standards of care, adhering to quality and best practice guidelines.
- Assess patient care needs, plan, deliver, and evaluate nursing care.
- Ensure safe delivery of prescribed medications and monitor effectiveness.
- Monitor patient clinical condition using tools and clinical judgment, escalating issues as required.
- Implement appropriate harm reduction measures to maintain patient safety.
- Work effectively within the multidisciplinary team (MDT), advocating for patients.
- Ensure safe admission, transfer, discharge, and handover of patient care.
- Communicate effectively with patients and their families, respecting their wishes.
- Demonstrate knowledge of legal frameworks related to consent, capacity, safeguarding adults, and vulnerabilities.
- Maintain clear and concise records.
- Supervise qualified and unqualified staff, contributing positively to their professional development.
- Undertake the role of shift leader once trained and competent.
Skills and Expertise:
- Registered Nurse qualification (Band 5).
- Experience in providing care for elderly patients.
- Excellent communication and interpersonal skills.
- Strong clinical judgment and problem-solving skills.
- Proven ability to manage patient care effectively.
- Knowledge of local policy and procedure.
- Understanding of legal frameworks related to consent, capacity, safeguarding adults, and vulnerabilities.
- Proficient in care documentation.
- Experience in medication administration.
- Knowledge of managing patients with multiple long-term health conditions and acute conditions.
