About the Role:
This Rehabilitation Therapy Assistant role focuses on supporting individuals in achieving their therapy goals and promoting their independence within the community. The role involves direct patient care and working collaboratively with other healthcare professionals to develop and implement personalized care plans.
Key Responsibilities:
- Supervise and support junior staff, providing guidance and mentorship.
- Work independently and take charge in the absence of qualified staff, making necessary judgments related to individual needs.
- Ensure effective engagement with the therapy plan and across relevant services.
- Plan and supervise individual or group therapy sessions.
- Provide practical support with daily living activities and promote independent living.
- Work collaboratively with key workers, care coordinators, and other support staff to implement care plans.
- Help individuals access resources, including benefits and welfare rights.
- Handle individual cash and valuables securely.
- Monitor progress, identify early signs of relapse, and alert appropriate staff.
- Assist in developing crisis/contingency plans and advanced statements.
- Maintain accurate and comprehensive case records.
- Communicate effectively and use tactful persuasion in diverse situations.
- Provide information on health promotion.
- Ensure individuals have a clear pathway of care across sectors.
- Attend and participate in training, development, and supervision sessions.
- Undertake other duties as required within the scope of the role.
- Adhere to health and safety legislation.
- Support Occupational Therapy and Physiotherapy staff in facilitating groups and completing home assessments.
- Support Occupational Therapy, Health and Wellbeing Coach, and Physiotherapy staff to follow a therapy plan, encompassing moving and handling, promoting independence, positive risk-taking, healthy lifestyle advice, and intervention.
Skills and Expertise:
- Proven experience in rehabilitation therapy.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Knowledge of relevant legislation and procedures.
- Excellent problem-solving and decision-making skills.
- Ability to handle sensitive information with confidentiality.
- Excellent written and verbal communication skills in challenging environments.
- Ability to manage multiple tasks and meet deadlines.
- Proficiency in relevant software/systems.
