About the Role:
Seeking a highly motivated Senior Practitioner (Band 7) for a 12-month secondment role in the Crisis, Assessment & Treatment Team (CATT) in Ealing. This role offers valuable experience within community healthcare, collaborating with other teams to respond to a variety of mental health needs. The focus is on delivering high-quality patient care and promoting positive community relationships.
Key Responsibilities:
- Conducting assessments and delivering brief home-based treatment for patients at risk of hospital admission.
- Collaborating closely with the Single Point of Access and other local services.
- Ensuring a comprehensive 24/7 service for patients.
- Maintaining robust gatekeeping procedures for inpatient wards and community recovery houses.
- Developing and implementing care pathways and care plans for patients.
- Providing clinical advice, support, and training to junior colleagues.
- Liaising with carers, statutory and non-statutory services, emergency services, and 3rd sector organizations.
- Improving mental health crisis response, reducing unnecessary A&E presentations.
Skills and Expertise:
- Proven experience as a Senior Practitioner (Band 7)
- Excellent assessment and clinical skills.
- Strong communication and interpersonal skills.
- Knowledge of mental health care pathways and crisis intervention.
- Ability to work within a 24/7 rota.
