About the Role:

Key Responsibilities:
- Develop and implement individualized support plans to meet the specific needs of each client.
- Provide direct care and support, including personal care, domestic tasks, and social activities.
- Monitor and record client progress, reporting any concerns or changes to relevant personnel.
- Collaborate with other support staff, therapists, and family members to ensure consistent and coordinated care.
- Actively participate in team meetings and training sessions to enhance professional skills.
- Promote client independence and well-being through encouragement and advocacy.
Skills and Expertise:
- Proven experience as a Support Worker or relevant experience.
- Understanding of the needs of vulnerable individuals and experience providing personalized support.
- Excellent communication and interpersonal skills.
- Strong problem-solving and decision-making abilities in challenging situations.
- Ability to work independently and as part of a team.
- First Aid and safeguarding qualifications (if applicable).
