About the Role:
Provide high-quality navigation and care support to beneficiaries of the Infected Blood Psychological Service (IBPS) in Hampshire. Conduct psychosocial assessments and interventions, and lead outreach programmes. Essential role in delivering multi-disciplinary support to beneficiaries.
Key Responsibilities:
- Plan, coordinate, and evaluate navigator services for IBPS beneficiaries.
- Assess beneficiaries’ emotional, social, practical, and financial needs, providing support or signposting.
- Respond independently to crises in collaboration with medical staff and the multi-disciplinary team.
- Liaise with statutory and non-statutory agencies, sharing specialist knowledge on the social needs of the Infected Blood community.
- Maintain accurate documentation of interventions, respecting beneficiary rights under the Freedom of Information Act.
- Gather and analyze data on support and preventative work facilitated by a national case management system.
Skills and Expertise:
- Social work qualifications and experience.
- Strong assessment and intervention skills.
- Proven experience in navigating complex social situations.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Knowledge of relevant legislation and procedures (e.g., Freedom of Information Act).
