About the Role:
As a Support Worker in Lancashire, you will play a crucial role in the opening of a new Supported Living service, providing high-quality enabling care to 7 individuals transitioning to independent adult placements. This is a fulfilling opportunity to help shape a new service and make a positive impact on the lives of those in your care.
Key Responsibilities:
- Implement person-centered support plans, promoting independence and managing risks while adhering to legal and best practice compliance.
- Develop engaging activity plans that promote autonomy and personal growth.
- Support individuals in maintaining healthy lifestyles and managing their health needs.
- Facilitate effective communication and maintain detailed, person-centered records.
- Oversee medication processes and support individuals in self-administration.
- Maintain a safe, clean-living environment and adhere to health and safety protocols.
