About the Role:

Support the physiotherapy and Occupational Therapy teams in delivering holistic interventions and self-management programs for patients with palliative diagnoses. Assist in evaluating patient progress and ensure effective communication for quality care.

Key Responsibilities:

  • Undertake clinical duties under therapist supervision.
  • Complete competencies in delivering treatment interventions for fatigue, breathlessness, and anxiety.
  • Assist the Multidisciplinary Team in delivering self-management groups.
  • Monitor patient exercise and rehabilitation programs.
  • Support patients with planned exercise regimes.
  • Assist with equipment ordering, fitting, and demonstration.
  • Explain therapy rationale and promote understanding.
  • Attend multi-disciplinary team meetings, case conferences, and handovers.
  • Complete competencies and teach therapeutic techniques.
  • Maintain accurate clinical records.
  • Collect data, statistics, and information.
  • Complete onward referrals and signposting.
  • Manage the therapy equipment store.
  • Monitor equipment maintenance and safety.
  • Assist qualified therapists in service delivery audits (if required).
  • Promote quality patient care.
  • Refer to qualified therapists when necessary.
  • Liaise with community teams for care records.
  • Identify and report safeguarding risks.
  • Adhere to Hospice incident reporting systems.
  • Assist in promoting the therapy service.

Skills and Expertise:

  • Understanding of palliative care.
  • Competencies in therapeutic techniques.
  • Experience in monitoring patient progress.
  • Excellent communication skills (written and verbal).
  • Ability to work effectively within a multidisciplinary team.
  • Knowledge of relevant Hospice policies and procedures.
  • Ability to maintain accurate records.
